The Exercise Referral scheme is a tailored physical activity programme that is designed to support inactive adults, who are living with a stable chronic health condition to become more active. Health conditions can include (but are not limited to) diabetes, high blood pressure and arthritis.

Individuals referred on to the programme will be able to access a 12 week tailored programme and will be supported by a qualified instructor. Currently the Exercise Referral Scheme is based out of Parklands Leisure Centre and individuals will be able to use the gym and swimming pool as well as receiving recommendations for activities such as walking, which can be done from home.

You will need to be referred on to the Exercise Referral Scheme by a medical professional, such as your GP. They will complete a referral form which will be sent to the Exercise Referral Coordinator and a member of the team will be in touch to discuss your referral and if suitable book in a consultation.

Upon completion of the Scheme your instructor will recommend suitable exit routes to allow you continue to participate in physical activity and improve your health.
